Inspector Notes
An unannounced, on-site renewal inspection was initiated on 5/7/2025 and completed on 5/7/2025, as a part of the licensure period. The on-site inspection began at 11:14AM and ended at 12:52PM. The inspector reviewed compliance in the areas listed above. There were 36 children present and 8 staff. The inspector reviewed 4 children?s records on-site 5/7/2025 and 3 staff records on-site 5/7/2025. This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-150-B
Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for Child #2 was not signed.
Plan of Correction: Provider will obtain signed immunization phone.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #2 TB was dated 2 months prior to date of employment and has been employed for one week.
Plan of Correction: Provider will obtain TB screening with 30 days of coming into contact with children.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #3 updated screening was due 3 months ago. The updated screening has not yet been completed.
Plan of Correction: Provider will obtain a current TB screening.

Hazardous substances must be in a locked place that prevents access by children. In the preschool classroom, a bottle of Dawn dish detergent was sitting on a sink, within reach of children.
Plan of Correction: Provider will lock hazardous substances out of reach of children.

When food is brought from home the food container must be clearly dated. None of the children's food containers in the preschool classroom were dated.
Plan of Correction: Provider will date food brought from home.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #2, enrolled for eight months, was missing the following required components: name of child?s physician.
Plan of Correction: Provider will obtain doctors name.
