Inspector Notes
The licensing inspector conducted an unannounced monitoring inspection Wednesday, April 19, 2023, from approximately 9:20 am to 10:50 am. This was a focused 60-day monitoring inspection to determine the family day home's compliance with licensing standards. There were 4 children in care, totaling 11 points with one caregiver supervising. Required postings were reviewed. The violations from the previous inspection were checked for corrections. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

2. The provider stated that she did not have a copy of child # 3?s physical.
Evidence:
1. Child 2?s record did not contain 1 of 2 emergency contact information.
2. The provider stated that she has ask the parent to provide a 2nd emergency contact, but the parent has not provided the name and contact information.
Evidence:
1. Child # 3 was observed with a sippy cup in her hand that was not labeled or dated.
2 . Child #2 was observed with a sippy cup in her hand that was not labeled or dated.
Evidence:
1. Child 3 had a start date of 2/23/2023 but did not have immunization documentation.
2. Provider stated that she had a copy of the immunizations but was unable to locate the document.
Evidence:
1. There was no menu observed to be posted.
2. The provider was asked about the menu. The provider stated that the menu had not been completed or posted.
