Inspector Notes
An unannounced inspection was conducted on August 30, 2023 in response to a self-reported incident regarding sight and sound supervision. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.
The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the self report is determined to be not valid.
