During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was conducted on January 28, 2025. The on-site inspection began at 1:30 p.m. and ended at 4:30 p.m. The inspector reviewed compliance in the areas listed above. There were 130 children present and 23 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from one state for Staff 1, who has been employed for more than a year and was working with children at the time of the inspection.
Plan of Correction: The out of state CPS for Staff 1 was received February 3, 2025.
Standard 8VAC20-780-510-B
Medication shall be given to a child only with written authorization from the parent. Two
emergency medications for one child did not have written authorization from the parent.
Plan of Correction: Staff will request written permission from the parent for the two emergency medications.
