Inspector Notes

An unannounced inspection was conducted on 8/5/25 from 9:50 am to 11:50 am in response to a complaint received by the licensing office on 7/29/25 relating to allegations of special care provisions and emergencies. The inspector reviewed 5 children?s records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

When children at the center have been exposed to a communicable disease, the parents shall be notified within 24 hours or the next business day of the center's having been informed.
1. There was a confirmed case of hand foot and mouth disease in the Toddler room on 6/2/25. The center was notified by the parent on 6/2/25.
2. The center did not notify parents on 6/3/25 of the confirmed case on 6/2/25. A notification was sent out on 6/4/25.
3. A confirmed case was reported to the center on 7/3/25 in the Preschool classroom. The center did not notify the parents of this confirmed case.
