Inspector Notes
An unannounced monitoring inspection was conducted today. All rooms were in ratio during the inspection. The playground was not inspected during the visit due to inclement weather conditions and was not in use.

Based on a review of records, one staff record did not contain a completed Sworn Disclosure Statement (SDS) prior to the first day of employment.
Evidence:
Staff A's record did not contain a completed SDS. (Date of Hire (DOH): 2/26/24)
Plan of Correction: A new Sworn Statement was completed during the inspection.

Based on a review of records, one staff record did not contain documentation of orientation being completed by the end of the first week of employment.
Evidence:
Staff A's record did not have documentation of orientation. (DOH: 2/26/24)
Plan of Correction: Orientation was done, the form was not signed. Signed and dated during the inspection.

Based on observation, hazardous materials were not stored locked.
Evidence:
A bleach water spray bottle was at the diapering area in the infant room, not stored locked.
Plan of Correction: Bottle was locked away during the inspection. Smaller bottles may be purchased to fit easier in the cabinet. Staff were reminded to lock up the bleach water once finished with diapering.

Based on observation, food brought from home was not labeled and dated.
Evidence:
Infant bottles were not dated for one child.
Plan of Correction: Bottles will be dated when they re labeled.

Based on a review of records, one staff record did not contain documentation of two references having been checked prior to employment.
Evidence:
Staff A's record did not contain documentation of a 2 required references.
Plan of Correction: References will be checked right away.

Based on a review of documentation, a written attendance was not kept in each group of children noting the arrival and departure as it occurs.
Evidence:
There was no written attendance in the infant room at LI arrival. There were 2 infants present.
Plan of Correction: Attendance was completed during the inspection. Staff were reminded to complete attendance as it occurs.
