Inspector Notes
An unannounced on-site code compliance inspection was initiated and completed on 4/2/2025. The on-site inspection began at 10:40 and ended at 11:55. The inspector reviewed compliance in the areas listed above. There were 4 children and 1 staff present in the classroom and 1 administrative staff present. The inspector reviewed 4 children?s records and 3 staff records at the main site. This inspection included document review, tour of the facility, interviews, and observation.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Staff #1 , #2 and #3 had completed a VA state fingerprint check but it was not completed by the required organization.
