Inspector Notes

A renewal inspection was conducted on September 14, 2023 from 10:30 A.M.-11:45 P.M. There were four children present for a total of eight points, ranging in ages from two years to four years of age, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and two caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the provider failed to ensure that all equipment and materials used by children shall be free from hazards.

Evidence:
1. In the outdoor play area there were was one trike that had a broken seat , and two of the trikes had cracks on the back of the trike.
2. The plastic basketball hoop was broken at the rim and cracked.
3. The provider verified that the equipment was broken and cracked.
Plan of Correction: The provider removed all broken equipment from the play area, and will monitor equipment that needs repairs will be removed from the children's area until repaired.
