During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 8, 2026 and completed on April 8, 2026. The on-site inspection began at 12:50 p.m. and ended at 1:55 p.m. The inspector reviewed compliance in the areas listed above. There were four children present and two caregivers; the point total was nine. The inspector reviewed two children?s records and two caregiver/household member(s) records on-site. The inspection included record review, tour of the facility, observations, and interviews. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-790-250-F
Caregivers who work directly with children shall, in addition to preservice and orientation training shall annually complete the department's health and safety update course. Staff #2 did not complete the department's health and safety update course in 2025.
Plan of Correction: Staff #2 will complete the department's health and safety update
course by the end of April and yearly.
Standard 8VAC20-790-430-B
The vendor shall maintain a record of the dates of the practice drills for one year. The licensee was unable to provide a record of lockdown drills, shelter-in-place drills, and emergency evacuation drills.
Plan of Correction: From now on documentation of emergency drills will be kept and updated as required.
Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Staff #1 only completed 11 hours in 2025 and Staff #2 completed 2 hours in 2025.
Plan of Correction: Staff #1 and Staff #2 will complete 16 hours of annual training by April 1, 2027.
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1 does not have one year, 18 month, or two year immunization records.
Plan of Correction: The parent of child #1 will be notified to provide the most current immunization record. In the future the licensee will ensure updated immunizations are obtained.
