Inspector Notes
A code compliance inspection was initiated on and concluded on February 9, 2024. There were two children present, ranging in ages from three and four years old with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision. A total of four children?s records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Based on record review and interview the center failed to ensure that each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
Evidence:
