Inspector Notes
A subsidy health and safety inspection was initiated and concluded on February 21, 2024.
There were two children present and two staff. A Code Compliance inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy
health and safety regulations. Only subsidy regulations that are not covered in Code of Virginia
are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy
regulations, and violations were documented on the violation notice issued to the facility.

Based on record review and interview the center failed to ensure that required background checks are obtained prior to employment.
Evidence:
1 - Four out of four staff records reviewed contain fingerprint background check results and sworn statements or affirmations dated after the documented date of employment.
a. Staff #1 has a documented date of hire as October 23, 2023. However, the fingerprint background check is dated November 30, 2023, and the Sworn Statement or Affirmation is dated November 15, 2023. Both background checks were completed after the date of hire.
b. Staff #2 has a documented date of hire as October 23, 2023. However, the fingerprint background check is dated November 28, 2023, and the Sworn Statement or Affirmation is dated November 20, 2023. Both background checks were completed after the date of hire.
c. Staff #3 has a documented date of hire as October 31, 2023. However, the fingerprint background check is dated December 5, 2023, and the Sworn Statement or Affirmation is dated November 17, 2023. Both background checks were completed after the date of hire.
d. Staff #4 has a documented date of hire as October 23, 2023. However, the fingerprint background check is dated November 29, 2023, and the Sworn Statement or Affirmation is dated November 20, 2023. Both background checks were completed after the date of hire.

Based on record review and interview the vendor failed to ensure staff are determined to be free of communicable tuberculosis (TB) within 30 calendar days of the date of employment..
Evidence:
1 - Four out of four staff records reviewed did not have documented TB screenings within 30 days of employment.
a. Staff #1 has documented date of hire as October 23, 2023 and a TB test dated December 7, 2023.
b. Staff #2 has a documented date of hire as October 23, 2023 and a TB test dated December 8, 2023.
c. Staff #3 has a documented date of hire as October 23, 2023 and a TB test dated January 8, 2024
d. Staff #4 has a documented date of hire as October 23, 2023 and a TB test dated April 27, 2023.
