Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/18/2025. A POC submitted after this date will not appear on the public website.

A tuberculosis (TB) screening is required every two years from the last screening.

Staff #3 updated screening was due approximately 2 months ago.The updated screenings have not yet been completed.
Plan of Correction: TB screening was completed.
Standard 8VAC20-790-600-D-1
All staff who work directly with children shall have within 90 days of the date of
employment current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Staff #4 received CPR certification approximately one month after the 90 day timeline.
Plan of Correction: Have staff to complete training within allotted time frame.

All staff who work directly with children shall have within 90 days of the date of
employment current certification in first aid appropriate to the ages of children in care.

Staff #4 received first aid certification approximately one month after the 90 day timeline.
Plan of Correction: Have staff complete training within allotted time frame.
Standard 8VAC20-790-700-A
Complaint related: Behavioral guidance shall be constructive in nature and age and stage appropriate and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On August 5th, 2025, Staff #5 lifted a 2.5 year old child off the ground by one arm to have the child stand up. Staff #5 was immediately corrected by licensing inspector and told that grabbing and picking children up by the arm is not the appropriate way to redirect a child.
Plan of Correction: Additional child management training will be taken by all staff.
