During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 10/2/2024 and completed on 10/2/2024 in response to a complaint received by the licensing office on 10/1/2024 relating to allegations of Supervision and Administration. The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included document review of Injury logs, policies and procedures,tour of the facility and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
340 A. When staff are supervising children, they shall always ensure their care, protection and guidance. A child received an injury while supervised by staff.
1. Child #1 (age 6) choked child #2 ( age 6) while in the care of staff #1 on September 30, 2024 at approximately 5:50 pm. A third child in the classroom attempted to help when staff #1 did not immediately respond.
Plan of Correction: We will retrain staff on appropriate supervision and activities.
Standard 8VAC20-780-340-C
340 C. During the stated hours of operation there shall always be one staff member who meets the qualifications of program leader or program director and an immediately available staff member.

Staff #1 was the only staff member present on 9/30/2024 from approximately 5:30 pm to 6:30 pm

Standard 8VAC20-780-340-D
340D. Each grouping of children shall have at least one staff regularly present who meets the lead teacher qualification. Staff #1 in the afterschool class does not have documentation of 12 hours of supervised programmatic experience.
Plan of Correction: We will document training.
Standard 8VAC20-780-550-P
The center did not maintain a written record of children?s serious injury and minor injuries in which entries are made the day of the occurrence.
1. Child #2 was choked by another child and no written documentation was made the day of occurrence ( 9/30/2024).
2. Staff #1 when asked by parent what had occurred stated ? it wasn?t a big deal?.
