During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 10/18/2024, as a part of the licensure period. The on-site inspection began at 10:05 am and ended at 12:15 pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 staff. The inspector reviewed 2 children?s records and 3 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employees first day of employment. Staff #1, who was employed for 1 day, did not have a completed national criminal background check.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming in contact with children. The screening for Staff #1 was not completed by day 1 of hire. Staff was employed 1 day.
Plan of Correction: We had an emergency need for a van driver. He only came for the afternoon to help me drive van. Staff is no longer employed. Only here that 1 day.

Electric outlets shall have protective covers that cannot be swallowed by children.
Electric outlet under step had prongs of cover in outlet but plastic cover was broken off in outlet.
