During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/10/2025, as a part of the licensure period, The on-site inspection began at 12:00 pm and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 staff. The inspector reviewed 2 children?s records and 2 staff records.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center must request a search of the central registry prior to the employee's first day of employment.
Staff A who had been employed with the facility for 12 days, did not have a central registry search requested.
Plan of Correction: I did not know it changed. I will send today by mail.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis screening must be submitted at the time of employment, before coming in contact with children, and shall have been completed within the last 30 days of the date of employment.
Staff #1 had no documentation and had been employed 12 days.
Plan of Correction: I will have him do.

Areas of the center were not maintained in clean condition.

The bathroom by the front door used by older children had not been cleaned or sanitized in the sink and toilet area. There were viable areas of dirt build up.

Electrical outlets shall have protective covers that cannot be swallowed by children.
Electrical outlet under the alphabet did not have covers.

Restrooms areas provided for children shall be equipped with soap, toilet paper and disposable towels within reach of children.

There was no soap or paper towels in the bathroom used by older children.
Plan of Correction: The person who does that is on break. They will do when they get back.

Cots when used by children for designated rest period shall have linens consisting of a top and bottom cover or a one - piece covering which is open on three sides.

Two children did not have a bottom cover during nap time.
Plan of Correction: We will ask mom for sheets at pick up. They take home on weekend.

Staff record requires documentation of two or more references as to character and reputation as well as competency before employment or volunteering.

Staff A who was employed for two weeks, was missing 2 of 2 required references.
Plan of Correction: We had 2 references but they were family. We will get new ones.
