During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 04/08/2025 and completed on 4/08/2025 with a local agency, in response to a complaint received by the licensing office on 04/07/2025 relating to allegations of Supervision. The inspector reviewed 1 children?s records and 2 staff records on-site This inspection included :document review , tour of the facility and routes, and interviews

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Administration and Staff Qualifications and Trainings, and are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employees first day of employment. The staff is supervised at all times by an individual with all required background checks completed within the last five years.

Staff A transported children to schools on April 7, 2025 from approximately 8:00 am to 8:50 am. Staff A was not supervised during that timeframe by a staff member with completed background searches. Staff A had been employed for 6 days.
Plan of Correction: We will never hire before the search report is arrived.
Standard 8VAC20-780-190-A
Program directors shall be at least 21 years of age and shall meet one of the following: A1-4

There is not a qualified director on-site.
Staff B stated that the director was on leave. Staff B is not director qualified and has not taken additional training to become director qualified.
Plan of Correction: We are looking for a qualified director.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Staff A did not maintain written attendance as Child A was dropped off to school on 4/7/2025.
Plan of Correction: We will prepare a written and very specific transportation policy and we'll have record of daily attendance.
