During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on 10/16/2025 and completed on 10/21/2025 with a local agency , in response to a complaint received by the licensing office on 10/15/2025 relating to allegations of administration and health and safety. The inspector reviewed records [on-site] and [electronically] on 10/16/2025 and 10/21/2025. This inspection included document review , tour of the facility, interviews, observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The center is required to obtain a copy of the results of the central registry before an employee can be alone with children.

Staff A was alone transporting children to school on 10/15/2025. Staff A's start date 10/14/2025.
Plan of Correction: I miss understood. I sent the Central registry before she started like we have discussed in the past. I was with her in the van on her first day. Her letter came the next day.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The vehicle used to transport children to and from school had an expired state inspection. The state inspection had been expired for for seven months.
Plan of Correction: We had the car inspected on 10/20/2025 and will email.
