Inspector Notes

A renewal inspection was conducted on 7/10/2023 from 11:10 am to 12:04 pm. No children were in care at the time. The Licensing Inspector reviewed three children?s records, one staff record, two household member records, and other requested program records. The LI conducted a walk-through of the childcare portion of the house.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Based on child record review, the provider failed to have a completed physical exam form in a child?s record.

EVIDENCE:
The record for child 3, who began attendance 3/23/2023, did not have documentation to show a physical exam had been completed before the first date of attendance. The record had no physical exam documentation.
Plan of Correction: The provider will locate the missing physical exam or have the parent get another physical to put in the record.

Based on child record review, the provider failed to have some required information in a child?s record.

EVIDENCE:
The record for Child 1 did not show who was allowed to pick up the child.
Plan of Correction: The provider will obtain the information and add it to the child's record.
