During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on and completed on December 2, 2024. The inspection started at 9:15am and concluded at 10:00am. There were no children present and 1 caregiver. The inspector reviewed 1 child's record and 3 caregiver/household members records on-site.This inspection included document review and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program

Standard 8VAC20-790-200-3
The caregiver record shall contain documentation of a current tuberculosis screening.
Caregiver has a TB screening that is over 7 months expired.
Plan of Correction: I will get my TB completed and make sure the results are in my record.
Standard 8VAC20-790-260-K
Stairs shall not be accessible to children less than two years of age and children older than two years of age who are not developmentally ready to climb or descend stairs without supervision.
Stairs to the upper level of the home were accessible to the children.
Plan of Correction: The gate located on the fourth step is broken. I will move the gate lower so no steps are accessible and make sure it is always locked.

REPEAT VIOLATION
Potentially poisonous substances, materials, and supplies shall be stored away from food in areas inaccessible to children.
Keep out of reach substances and materials were accessible to children in the bathroom and front hallway.
Plan of Correction: I was cleaning all weekend and will make sure everything is away before I go pick up the child.
