During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated on and completed on January 9, 2025, in response to a complaint received by the licensing office on December 26, 2024 relating to an allegation related to non compliance of the Vendor Agreement The inspector reviewed 2 children?s records and 3 caregivers records on-site. This inspection included document review, tour of the day care area, and interviews.The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to supervision and background checks and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 30, 2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.040-A
1.Household Members 18 years of age and older must obtain a completed sworn statement prior to the household member moving in. Adult household member must obtain a completed national criminal background check prior to moving in.

2. Household members 14 years of age and old must obtain the results of a central registry search by the end of the 30th day of moving in.

3.Adult household members must obtain background checks from any state in which the individual has resided in the preceding five years.

Household members #1, 2 and 3 do not have a completed national criminal background check and have lived in the home for more than five months.

The provider shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

REPEAT VIOLATION
Stairs shall not be accessible to children less than two years of age and children older than two years of age who are not developmentally ready to climb or descend stairs without supervision.
Stairs located in the childcare area were accessible to the children in care.
Plan of Correction: Repeat plan of correction still in progress. Will be fixed.

A caregiver shall be physically present on site and provide direct care and supervision of each child at all times.
When the inspector arrived at 10:00 a.m., the provider was not present in the home and children #1 and #2 were already present in the home. The provider arrived at 10:06 a.m.
Plan of Correction: I'll be physically present at all times.
Standard 8VAC20-790-360-B
Children shall be protected from materials that could be swallowed or present a choking hazard. Toys or objects less than 1-1/4 inches in diameter and less than two inches in length shall be kept out of reach of children less than three years of age.

Children less than three years of age had access to various toys and objects less than 1 1/4 inches in diameter, including small building blocks.
Plan of Correction: Small toys were removed immediately.
