During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated and completed on 02/25/2025 with a local agency in response to a complaint received on (date) relating to allegations of child abuse.
The inspector reviewed 3 children?s records on-site. This inspection included document review, tour of the care area, and interviews.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to supervision, home maintenance, nutrition, handwashing, and records, and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today. A POC submitted after this date will not appear on the public website.
Violations
9The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
No attendance record was completed for the month of February. The attendance records for January 2025 and December 2024 were incomplete and were not consistent in documenting the arrival and departure of the children.
The following unsanitary and unsafe conditions were observed on the day of inspection:
* Heavy smell of smoke (cigarrete) inside the home.
* Generalized areas of the floor with crumbs and ripped pieces of paper/cardboard.
* A small table accessible to the children showed signs of not being clean like crumbs, straw wrappers, empty cups, ripped pieces of paper, dirt stuck on the table.
* Boxes and other materials stacked along the hallway that leads from the entrance door to the care area. These may become unstable and fall on the children.
A caregiver shall provide direct care and supervision of each child at all times.
The Vendor reported that on February 6, 2025, after demonstrating in front of the children the use of a flyswapper to sing along to a song, she gave the flyswapper to the children for them to use. Afterwards, the vendor sat at a nearby table with her back turned to the children while they played with the flyswapper. During this time, Child #1 was struck on his face with the flyswapper which resulted in a visible welt on the right side of his face.
The incident of January 30, 2025 on which Child #1 (3 years old) arrived home with scratches on his upper chest was not documented . An injury report was not filled out and the parent's signature requested.
The Vendor stated that on February 6, 2025, she utilized a flyswatter as a prop to sing along to Ms Rachel's song "Shoo Fly". The Vendor demonstrated using the flyswatter and tapping on the children. The Vendor then allowed the children to use the flyswatter unsupervised and in a manner that is not intended to be used which resulted in Child #1 being injured.
The Vendor stated that Child #4 and Child #5 did not attend public school the date of inspection because they were showing symptoms like: being "feverish" and having "stuffy noses". Despite the children having symptoms, they were in the care area playing with other children.
Caregiver #1 did not wash Child #2's hands after their diaper was changed.
Caregiver #1 was observed changing the diaper of Child #2 and did not wash her hands afterwards.
Two cans of baby formula were being stored on the sink in the bathroom of the care area.
