Inspector Notes

An unannounced complaint investigation was conducted on November 8, 2023, in response to a complaint made to our office regarding General Qualifications for Caregivers and Qualifications and Requirements for Providers and Substitute Providers.

Based on interviews conducted, observations and record review, the evidence supports the claim regarding documentation for substitutes but does not support the other claims. The Licensing inspector observed children indoor play activity and interactions between the Provider/caregivers and children.

Please submit plan of corrections within 5 business days in order for it to be included on the website.

Standard 8VAC20-800-140-D
Based on interviews and record review, documentation providing the time of arrivals and departures on each day that the substitute provider works, was not in compliance.
Evidence:
The Provider stated there was no record kept for October 18, 2023 and October 31, 2023.
Plan of Correction: I will record the time of arrivals and departured on each day the substitute provider work.
Standard 8VAC20-800-50-A
Based on attendance records, the provider did not maintain a written record of children in attendance each day.
Evidence:
1. There was not record for attendance for enrolled children October 30 and October 31, 2023.
Plan of Correction: Attendance records were on the back of attendance sheet. I will check carefully.
Standard 8VAC20-800-50-B
Based on interview, the provider did not maintain records in the home and made accessible to the department's representative.
Evidence:
1. The Provider stated the attendance records for October 30 and 31, 2023 were kept but she was unable to find it while the Licensing Inspector was present. Therefore the attendance sheet was unavailable for review for October 30 and 31, 2023.
Plan of Correction: Attendance records was on the attendance sheet. I will check carefully.
