During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on and completed on 07/03/2025 in response to a complaint received by the licensing office on 06/27/2025 relating to allegations of supervision. The inspector reviewed 1 children?s record and 1 staff record. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/10/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-560-A
A caregiver shall be physically present on site and provide direct care and supervision of each child at all times. Direct care and supervision includes awareness of and responsibility for each child in care, including being near enough to intervene if needed.

Staff #1 did not provide direct care and supervision to child #1. Child #1 informed Staff #1 that they needed to use the restroom. Staff #1 instructed the child to go ahead upstairs. Child #1 had an accident and did not make it to the restroom in time, however Staff #1 was not able to provide assistance as they were downstairs cleaning. Child #1 was seen, by their parent who was picking up at the time, standing at the top of the stairs, attempting to change their own clothes.
Plan of Correction: We will make sure a caregiver stays near a child using the restroom moving forward, so that we may intervene as needed.
