Inspector Notes

An announced initial inspection was conducted on March 1, 2023 from approximately 9:50 AM to 11:40 AM. An additional inspection was conducted on March 20, 2023 and March 22, 2023 to review the infant and preschool outdoor play areas. Currently, the center's playground area is under construction and will not be available for use until construction is complete. There were no children in care during this inspection as the center has not yet opened for child day care purposes. The center is requesting a licensed capacity for 176 children ages six weeks through 5 years, 11months. The center has submitted all the required application materials. During the inspection, compliance with some standards could not be determined since the center is not yet in operation. Compliance with these standards will be determined during unannounced visits once the program is in operation. Though some standards of care could not be determined during the initial study, the center is in substantial compliance with the standards that could be determined. A six-month conditional license will be recommended. During the conditional period, the center will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Based on record review, the center failed to obtain an updated Sworn Statement for an employee whose Sworn Statement was dated more than 90 days prior to the date of the initial application.

Evidence:
The record of staff #3, had a sworn statement dated 09/30/22 exceeding the 90 days prior to the date of initial application.
