During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on January 28, 2025 and completed on February 19, 2025 in response to a self-report received by the licensing office on January 24, 2025 relating to an injury requiring outside medical care. The inspector reviewed one child's record and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 28, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their protection and guidance.

On 1/24/25, staff #1 was changing a diaper and staff #2 was feeding an infant. Child #1 (9 months old) followed staff #1 to where she was changing a diaper. Staff #1 and staff #2 stated that child #1 pulled up on the back of a child size chair resulting in child #1 falling backwards on the tile floor and the chair falling on top of child #1 who was observed crying and bleeding from his mouth. Staff #1 was able to reach the chair where she was standing and pull the chair off child #1. Staff #2 picked up child #1 and provided first aid. Child #1 was seen by a dentist which resulted in a tooth extraction due to the injury. Staff #1 and staff #2 reported that earlier that morning child #1 was observed pulling up on a trash can, resulting in child #1 rolling down to the ground on his back without injury.
Plan of Correction: The teachers received additional supervision training that entailed developing the foresight to precent injuries based on more insightful and responsive supervision. (ie. remaining within arms reach of children that are pulling up or taking first steps). Using proactive measures as such in the classroom will reduce the risk exposure for children.
