During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on August 27, 2025 and completed on September 3, 2025 in response to a self-report received by the licensing office on August 26, 2025 relating to supervision. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 16, 2205. A POC submitted after this date will not appear on the public website.
Violations
3When staff are supervising children, they shall always ensure their care, protection, and guidance.
1. On August 26, 2025 child #1, age 3 years, was left alone and unsupervised on the playground. Child #1 was discovered holding the hand of another child?s parent through the playground fence when staff #2 was entering the playground with their class.
2. Staff #1 did not realize child #1, age 3 years, was left alone and unsupervised on the playground until after entering the classroom when staff #2 brought child #1 back in from the playground.
On August 26, 2025, child #1, age 3 years, was left alone and unsupervised on the playground.
1.The center?s policy titled ?Supervision Policy? states ?Name-to-face checks or threshold counts must be conducted and documented every time the children move from one location to another, such as when the children move from the classroom to the gym, from the classroom to the playground and from the playground to the classroom?.
2. Staff #1 did not realize child #1, age 3 years, was left alone and unsupervised on the playground until after entering the classroom, counting the number of children two times, and grabbing for the table to complete name to face check when staff #2 brought child #1 to the classroom from the playground.
