During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on March 18, 2026 and completed on March 18, 2026, as a part of the provisional licensure period. The on-site inspection began at 11:28 AM and ended at 3:30 PM. The inspector reviewed compliance in the areas listed above. There were 141 children present and 29 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 27, 2026. A POC submitted after this date will not appear on the public website.

Children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine. Staff shall wash their hands with soap and running water before and after a diaper change.

1. Child #3's hands were not washed after diapering.
2. Staff #A did not wash their hands after diapering.
Plan of Correction: Retrain staff on the handwashing practice for both children & teachers. Spot check throughout the day.
