Inspector Notes
An unannounced, on-site monitoring inspection was initiated on April 24, 2025 and completed on April 24. 2025. The on-site inspection began at 11:45am and ended at 1:25pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/14/2025 . A POC submitted after this date will not appear on the public website.

Licensees shall at all times afford the Superintendent reasonable opportunity to inspect all of their records. The record for staff #2 and staff #3 were not made available for inspection.
Plan of Correction: Files will be printed, filed and made available.

REPEAT VIOLATION
The center must request a search of the central registry prior to the employee's first day of employment. Staff #1, who had been employed for 1 year, did not have a central registry requested.
Plan of Correction: Central registry was done before employment. The paper was misplaced but found and filed.

REPEAT VIOLATION
Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance. The record for child #2 who has been enrolled for over seven months did not contain documentation of a completed physical.
Plan of Correction: The physical for child #2 was updated and filed.
Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. The tuberculosis screening in the record for staff #1 was expired for approximately two months.
Plan of Correction: The TB test for staff #1 has been scheduled for 5/20/2025
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. The children were sleeping in classrooms with inadequate lighting, which prohibited staff from supervising.
Plan of Correction: Additional lighting was purchased the day of the visit.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. There was a bottle of hand sanitizer and a bottle of whiteout on a desk in the preschool room. All items had the ?keep out of reach of children? label and were unlocked and accessible to children.
Plan of Correction: Hazardous substances was removed as soon as it was brought to the director's attention. Staff was retrained.

There shall be at least 12 inches of space between occupied cots, beds, and rest mats. Multiple occupied cots in the center had less than 12 inches of spacing in between them.
Plan of Correction: Cots will have 12 inches of space going forward.
Standard 8VAC20-780-570-B
Bottles shall not be used while the child is in his designated sleeping location. Each infant had their bottle in their crib.
Plan of Correction: Bottles were removed and staff was retrained.

REPEAT VIOLATION
Children?s records shall include the phone number of two designated people to call in an emergency if a parent cannot be reached. The record for child #1 who has been in care for over three months did not have the phone number for one emergency contact.
Plan of Correction: The emergency contact # was added to the registration form.
