Inspector Notes

An unannounced renewal inspection was completed from 10:25am until 12:10. There were 8 children present with two providers supervising. The point total was 22. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 1 staff record was reviewed.

Safe sleep procedures were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-120-A
Based on record review, a caregiver did not have the required paperwork.
Evidence:
1. There was an assistant working and did not have letters of reference; two are required from non-relatives.
Plan of Correction: References will be gotten for the assistant.

Based on record review, the provider did not have assistant orientation documentation.
Evidence:
1. FIle #1 was reviewed and did not have orientation documentation.
Plan of Correction: Orientation will be completed for the assistant.
Standard 8VAC20-800-240-A
Based on observation the areas of the family day home were not maintained in a clean, safe, and operable condition.
Evidence:
1. The wall leading to the bathroom in which the children have access to has an of approximately 4 by 12 inches that the plaster and paint have been scratched.
Plan of Correction: The wall will be repaired.

Based on observation, there were cleaning supplies accessible to children.
Evidence:
1. In the bathroom the children use there was a spray bottle of cleaner and a bottle of h. peroxide under the sink unlocked.
Plan of Correction: Corrected during the inspection.
Standard 8VAC20-800-530-F
Based on observation, there were items in the crib with an infant sleeping.
Evidence:
1. The inspector observed an infant in the crib asleep with a bobby pillow, a doll and a fleece blanket.
Plan of Correction: The provider removed the items from the crib.
Standard 8VAC20-800-960-E
Based on observation, bottles were not labeled.
Evidence:
1. Two infants were in care and the bottle were not labeled; name and date are required if more than one infant is in care.
Plan of Correction: Bottles will be labeled when there are more than 2 infants in care.
