During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on and completed on 8/5/2025. The on-site inspection began at 9:00 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were six children present for a point total of 8 and the Provider. The inspector reviewed five children?s records and the provider and three assistant records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening.

The Provider, Assistant #1, Assistant #2 and Assistant #3 did not have a current tuberculosis screening in their file.
Plan of Correction: Provider is scheduling an appointment for TB screenings and will provide documentation of screening once completed.
Standard 8VAC20-800-210-A
Caregivers shall obtain minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Assistant #1, #2 and #3 did not have the required sixteen hours of training in 2024.
Plan of Correction: The Provider will ensure all assistants complete the required sixteen hour annual training.
Standard 8VAC20-800-510-A
A child shall be provided with an individual crib, cot, rest mat, or bed for resting or napping.

There were cots or rest mats provided during nap time.
Plan of Correction: Cots or rest mats will be purchased and individually assigned for each child.
Standard 8VAC20-800-800-B
The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan.

The current emergency plan had not been reviewed since 2023.
Plan of Correction: The Provider will update the emergency plan within five business and document the review.
