Inspector Notes

An unannounced monitoring inspection was conducted on 03/27/2024 from 10:10 a.m. to 11:40 a.m. There were 16 children present, ranging in ages from three to five years old with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and one staff record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the center failed to maintain a separate record for each child which contained the required information.
Evidence:
1. Child #1, #2 and #4?s record contained a place of employment for one parent but did not indicate which parent the employment information was for and did not contain employment information for the second parent.
2. Child #3?s record only contained employment information for one parent.
3. Child #5?s record did contain employment information for either parent.
Plan of Correction: Have my team help guide me to where to gather all information and submit to inspector.
