During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 9/10/2025. The on-site inspection began at 10:15 am and ended at 1:25 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and three caregivers; the point total was 31. The inspector reviewed five children?s records and five caregiver/household member(s) records on-site.

Standard 8VAC20-770-40-D-5
Household members 14 years of age and older must obtain the results of a central registry search by the end of the 30th day of becoming 14 years old.
Household Member #2 did not have central registry results during the inspection and is over the age of 14 years.
Plan of Correction: The request was made five months ago. A new request was sent in 9/11/25.
Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.
An updated TB screening for Caregiver #1 and household member #1 was not completed and was over due by seven months.
Plan of Correction: TB tests/screens have been requested and read.
Standard 8VAC20-800-30-A
The provider shall ensure compliance with these standards and the terms of the current license issued by the department and with relevant federal, state or local laws, and other relevant regulations.
Per the local Zoning Administrator the provider can have no more than five children under the age of 2 1/2 years. During the inspection, seven children in care were under 2 1/2 years of age.
Plan of Correction: Contact with be made with the zoning administrator to request if this can be changed. If it is not changed, provider will come into compliance.
Standard 8VAC20-800-530-B
Cribs when being used by infants shall have a tight-fitting bottom cover.
The crib sheets in the occupied cribs were not tight-fitting.
Plan of Correction: Replaced crib sheet with a tight fitting crib sheet. Will ensure that all sheets on cribs are tight fitting.
Standard 8VAC20-800-540-A
High chairs shall be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.
Two of the seven occupied high chairs did not have the safety strap securely fastened.
Plan of Correction: Reminded staff to make sure all safety belts are secured.
Standard 8VAC20-800-920
When meals or snacks are provided by the family day home, the menu for the current one-week period shall dated.
The menu posted for this week was not complete and was not dated.
Plan of Correction: The menu was completed for the entire week.
Standard 8VAC20-800-960-E
Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.
The infant bottles were not labeled with the child's full name or dated. There was more than one infant in care.
Plan of Correction: Provider and staff will ensure that all bottles are labeled with the child's name and date.
