Inspector Notes
An unannounced monitoring inspection was conducted from 10:15 am to 12:30 pm. At the time of entrance, 46 children were in care with 13 staff members supervising. Additional staff were present for administrative duties. The sample size consisted of 5 children?s records and 5 new staff records. Children were observed eating lunch, participating in a in-house field trip and playing freely. Areas of non-compliance are identified on the violation notice.
Violations
6Evidence: The following was found during record review:
1. The record for staff #1 (start date 4/17/2023) did not have a child abuse/neglect registry search for a state the individual has lived in within the past 5 years.
2. The record for staff #2 (start date 5/30/2023) did not have a criminal history search for a state the individual has lived in within the past 5 years.
3. The record for staff #3 (start date 4/24/2023) did not have a criminal history search and child abuse/neglect registry search for 2 states the individual has lived in within the past 5 years.
2. Staff #2 proper paperwork was sent and in communication with someone in DES making sure it is completed properly.
3. Staff #3 paperwork was sent to portal and making sure proper background checks are done.
Evidence: The record for child #5 (start date April 24, 2023) did not have a physical.
Evidence: The following was found during record review:
1. The record for staff #2 (start date 5/30/2023) had a TB screening dated 2/23/2023.
2. The record for staff #5 (start date 5/30/2023) had a TB screening dated 6/1/2022.
Evidence: Staff #3 was present with 4 children in the Infant B classroom. There was not documentation of program leader or program director qualifications in the staff record. Director stated the individual is not program leader qualified.
Evidence: The following was observed:
1. Diapering areas in the facility had a open basket for soiled linens when a covered receptacle is required.
2. The changing table in the Toddler A classroom had toys and puzzles sitting on the surface.
Evidence: The record for staff #4 (start date April 17, 2023) did not have documentation of orientation training. Director stated the training was completed but the form was not available in the record.
