Inspector Notes

An unannounced monitoring inspection was initiated on October 26, 2023 and concluded on November 16, 2023 in response to a self-report made by the facility on October 23, 2023 regarding allegations in the area of programs. Licensing Inspector conducted classroom observations, interviewed staff and reviewed facility record keeping. One staff record and one child record were reviewed. A violation was found and is noted on the violation notice.

Based on interviews with staff and review of recordkeeping, facility did not ensure staff do not commit any forbidden actions.

Evidence: On October 20, 2023, between approximately 2:30 pm ? 3:00 pm, staff #1 was observed roughly handling child #1 (16 months old). Child #1 was crawling off her cot and staff #1 pulled child by her ankles approximately 16? back onto her cot. Staff #1 was then observed picking child #1 up by the outside of her upper arms and placing her on the changing table.
Plan of Correction: When realizing the incident, the teacher was let go immediately. The immediate steps were followed such as informing the parents, informing CPS and licensing. All teachers in the building were retrained on properly handling children and using the appropriate tone.
