Inspector Notes

An unannounced complaint inspection was initiated on June 5, 2024 and concluded on June 27, 2024 in response to allegations involving administration, staffing/supervision, programs and special care provisions. Licensing Inspector reviewed standards related to TB tests, staff training, daily activities, programs, supervision and facility documentation. Classrooms observations were conducted and collateral interviews were conducted. Violations were found related to staffing/supervision therefore that portion of the complaint is deemed valid. There was not a preponderance of evidence to support the allegations involving administration, programs and special care provisions therefore that portion of the complaint is deemed not valid. Based on interview, facility did not ensure there is is at least 1 staff per 4 children when the youngest in the group is of infant age (birth to sixteen months old).

Evidence: On approximately June 14, 2024 during the morning hours, 2 staff were present with 11 children in the toddler classroom for approximately 20 minutes. Child #1 (age 14 months) was the youngest in the group.
Plan of Correction: Administration will ensure that all classrooms are in ratio, based upon the youngest child present, by issuing a true up count ever 15 mins and documenting count.

Based on interview, facility did not ensure there is at least 1 staff per 10 children, ages 3 years up to school age.

Evidence: During the week of June 17th during the morning hours, 1 staff was present with 14 children ages 3 - 5 years old for approximately 30 minutes.
Plan of Correction: Administration will ensure that all classrooms are in ratio, by issuing a true up count ever 15 mins. and documenting count.
