Inspector Notes
An unannounced inspection was initiated on October 3, 2024 and completed on October 3, 2024 in response to a complaint received by the licensing office on September 23, 2024 relating to allegations of supervision, programs and special care provisions/emergencies. The inspector reviewed 6 child records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 14, 2024. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-350-B-3
(Repeat violation) The required ratio for children age 2 years is one staff per eight children. According to an Accident/Incident Report completed on 8/20/2024, Staff #2 was present with nine children in the Preppers Room (children 2 years old).
Plan of Correction: All classrooms were in ratio during 10.3 inspection. Ratio checks are conducted hourly.
Standard 8VAC20-780-500-A
Children?s hands shall be washed with soap and running water after toileting. Children's hands also shall be washed with soap and water before and after eating meals or snacks unless disposable wipes are used. Staff shall wash their hands with soap and running water before and after a diaper change. Staff changed a child?s diaper in the toddler A room and did not wash the child's hands or staff's hands with soap and water. Staff in the toddler A room used a wet paper towel to wipe children's hands after eating breakfast. It was not soap and water or disposable wipes. Staff in the prepper A room and preschool 1A room did not wash children's hands with soap and water or disposable wipes before eating breakfast.
Plan of Correction: All teachers were trained on proper handwashing and cleaning procedures in the classroom.

The center shall notify the parent by the end of the day of any known minor injuries. An Accident/Incident Report for Child #5 documented an injury occurring on 9/19/2024 and parent was notified on 9/23/2024. An Accident/Incident Report for Child #8 documented an injury occurring on 7/8/2024 and parent was notified on 7/9/2024. An Accident/Incident Report for Child #9 documented an injury occurring on 9/6/2024 and parent was notified on 9/11/2024.
Plan of Correction: Parents are contacted same day of incidents and reports will be provided at pick-up.

The center shall maintain a written record of children's injuries and include required information. Seven of nine Accident/Incident Reports did not have documentation of all required information to include parent notification (date/time/method), future action to prevent reoccurrence and a second signature.
Plan of Correction: Center management will contact parents same day of incident and will ensure documents are completed prior to filing.

Tables shall be sanitized before and after each use for feeding. The tables were not sanitized in the prepper A room and preschool 1A room before children were served breakfast.
Plan of Correction: All teachers were training cleaning procedures in the classroom.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #5, who has been actively working at the center for the past 3 months, did not have any completed reference verifications on file.
Plan of Correction: Reference checks were completed. Will ensure checks are provided prior to start date.
