During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on December 19, 2024. The on-site inspection began at 10:12 am and ended at 2:20 pm. The inspector reviewed compliance in the areas listed above. There were 88 children present and 20 staff. The inspector reviewed 8 children?s records and 8 staff records on-site This inspection included document review, tour of the facility, interviews, observations, and measurements. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 31, 2024. A POC submitted after this date will not appear on the public website.

The center must request a search of the central registry prior to the employee's first day of employment. The record for staff #4, who had been employed for three months, did not have documentation of a central registry search requested. The record for staff #6, who had been employed for two weeks, did not have documentation of a central registry search requested.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #7, who is working with children, has not yet completed this course which is overdue by 1 month.

Standard 8VAC20-780-500-B
Disposable diapers shall be disposed in a leakproof system that is either foot-operated or used in a way that neither the staff member's hand nor the soiled diaper touches the exterior surface of the disposal system. Diapering surfaces can only be used for diapering or cleaning children. A soiled diaper was observed disposed of in a swivel top trash can in a classroom bathroom. Two balls and artwork were observed laying on a diapering surface in a classroom where children are diapered.

(Repeat violation) Staff records must have documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment and documentation to demonstrate the individual possesses the orientation training required. Staff #3, who has been working at the center for the past 3 months, did not have any completed references and orientation training on file. Staff #4, who has been working at the center for the past 3 months, did not have orientation training on file. Staff #5, who was observed working with children, did not have documentation of date of employment, completed references and orientation training on file.
