An unannounced inspection was initiated and completed on October 8, 2025 in response to a complaint received by the licensing office on September 15, 2025 relating to allegations of physical plant. This inspection included document review, tour of the facility, interviews, observations and measurements. The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued
