Inspector Notes

An unannounced mandated inspection was initiated and completed on 5/6/2024 from 1:00am until 3:35 by two inspectors. There were 47 children and 14 staff present. Five children?s records and five staff records were reviewed.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

An exit conference was held with the director and administrator. Information gathered during the inspection determined non-compliance with applicable standards or law and violation (s) were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on record review, the center failed to obtain out of state child abuse and neglect results and sex offender registry check within the required time frames.
Evidence:
1. Staff #5 was hired 3/19/2024 and the required out of state child abuse and neglect registry check had not been completed by the end of the 30th day of employment. Out of state sex offender had not been completed prior to the first day of employment.
Plan of Correction: The sexual offender registry checks were completed during the inspection. The put of state central registry checks will be completed.

Based on observation, the center did not have the prescription label for a medication.
Evidence:
1. Child #6 had a medication at the center and it did not have a prescription label.
Plan of Correction: The parent was contacted and decided to take the inhaler home.

Based on record review, staff records did not contain all the required information.
Evidence:
1. Staff #1 and staff #2 did not have emergency addresses in the record.
Plan of Correction: The director will get the addresses from the employees.
