During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site inspection was initiated and completed on 3/3/2025. The on-site inspection began at 11:20 and ended at 1:45. The inspector reviewed compliance in the areas listed above. There were 40 children and 14 staff present. The inspector reviewed 5 children?s records and 5 staff records at the main site. This inspection included document review, tour of the facility, interviews, and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #3, employed for approximately 6 weeks, did not have documentation of requesting a central registry check or a sex offender registry check from a state.
Plan of Correction: The sex offender check was completed and corrected during the inspection. The out of state central registry check will be completed and submitted.
Standard 8VAC20-780-280-B
Repeat violation: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

In the staff bathroom on the fist floor, there was a can of Lysol, air freshener and bleach cleaner under the sink that was not locked.
Plan of Correction: Corrected during the inspection. All hazardous substances will be locked.
Standard 8VAC20-780-440-A
Cribs, cots, rest mats or beds shall be provided for children during the designated rest period and not occupied by more than one child at a time.

Children in the pre-k classroom did not have cots or rest mats during nap time.
Plan of Correction: Mats are at the center and will be used during nap time.
Standard 8VAC20-780-60-A
Repeat violation: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain information such as emergency contacts and chronic conditions.

Child #1, #2 and #3 were missing the required information.
Plan of Correction: The required information will be gotten from the parents.
