During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced on-site renewal inspection was initiated and completed on 8/4/2025. The on-site inspection began at 10:50 and ended at 2:20. The inspector reviewed compliance in the areas listed above. There were 41 children and13 staff present. The inspector reviewed 4 children?s records and 4 staff records. This inspection included document review, tour of the facility, interviews, and observation.
Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
7The infant rooms had Lysol stored on the back of the doors.
An area outside of the infant room that is used as a play area has a patio area 14 inches tall in which children could fall from. There is a railroad and a couple of protruding stakes
There was no written authorization for a nonprescription medication for child #1. The medication had not been given. A form had been completed but it was not the correct form. It had never been administered. There was no authorization for Child #2 to receive a non-prescription medication, it had not been administered.
Child #4 and child #5 (under the age of 2) had non-prescription medication that had been administered not consistent with the manufacturer's instructions for age, duration, and dosage.
There was non prescription medication for child #1 that was not in the original container with the direction label attached.
Child #4 had an authorization for a medication that was on site that expired 6/2/2025.
Diaper ointment was being stored in a lower cabinet which was accessible to children.
