Inspector Notes
A monitoring inspection was conducted on 06/13/2024 from approximately 9:56 a.m. to 1:10 p.m. There were 18 school age children present and 4 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five child records and four staff records were reviewed.
The information gathered during the inspection determined violations with applicable standards or law. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.
Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

The record for staff S4, employed 03/04/2024 did not have results of a national criminal
background check prior to the first day of employment. The results of the national criminal background check was dated 04/08/2024.

Staff record for S4 documented on the sworn statement having lived out-of-state in the
last five years. The record for S4, employed 03/04/2024 did not contain a sex offender searches for two states in which the individual lived in the preceding five years, the results of the out-of-state child abuse and neglect searches from one state, and the results of a criminal background check for one state.
Evidence:
The center did not have a carbon monoxide detector onsite.
Based on staff record review, the provider failed to have central registry findings within 30
days of employment.

The record for S4, employed 03/04/2024 did not contain findings of a central registry search.

The MAR for C6 documented a medication being administered by staff on 05/29/2024. The child record did not contain written authorization from the parent.

backpack in an unlocked locker in the school hallway. Licensing inspector observed the medication in the child?s backpack in the unlocked locker without a safe locking method that prevents access by children.

Licensing inspector observed multiple lunch boxes and water bottles that were not labeled with the children?s names.
Evidence:
The record for child, C2 did not document: first day of attendance; child?s address; parents address and phone numbers; two designated people to contact in case of an emergency if the parent cannot be reached; and any health problems or allergies.
Based on record review, the provider did not keep the required records for one of four staff.
Evidence:
The record for S2, did not contain documentation to demonstrate orientation and
training required by 8VAC20-780-240-C, 8VAC20-780-240-D, and 8VAC20-780-240-E. The record did not contain a completed orientation and training log and did not document information about any health problems that may interfere with fulfilling the job responsibilities.
