Inspector Notes
An unannounced, renewal inspection was completed on 07/17/2025. The on-site inspection began at 10:12 a.m. and ended at 1:00 p.m. The inspector reviewed compliance in the areas listed above. There were 15 children present and 3 staff. The inspector reviewed 5 children?s records and 3 staff records. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/30/2025. A POC submitted after this date will not appear on the public website.
Violations
8Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of C5, who had been enrolled for more than 1 month.
Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record and physical examination for C2 were not signed.
The center shall maintain written records of children?s injuries that contains all of the
required information. Injury reports dated 06/16/2025, 06/30/2025, and 07/11/2025
did not document the time of parental notification.
When food is brought from home the container must be clearly labeled with the child?s name
and date. Multiple thermos bottles were observed in the classroom were not labeled with the children?s names and dates.
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for C2, enrolled for more than a month, was missing the addresses for both emergency contacts. The records for C1 and C5 enrolled for more than one month were missing the written agreements between the parent and the center as required by 8VAC20-780-90.
Staff records shall include information about any health problems that may interfere with fulfilling the job responsibilities. The staff record for S2 did not include this information.
