Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on completed on 12/11/2024. The inspection started at 1:30 pm and concluded at 4:45 pm. There were 17 children present and five staff. The inspector reviewed one child?s record and 10 staff records on-site

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/19/2024.

The center is required to obtain a completed sworn statement prior to the employee's first day of employment. There are no sworn statements available for Staff # 2 & 3, who have been working, 12-20 months..

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1, 3 & 10 had no documentation and had been employed for 20 months, Staff #2 had been employed for one year, Staff #4 had been employed for eight months, Staff #5 & 6 had been employed for one month, Staff #8 had been employed for one two months.
