During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 10/23/24 and completed on 10/23/24, as a part of the licensure period. The on-site inspection began at 11:00am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were five children present and two caregivers; the point total was 14. The inspector reviewed five children?s records, two caregivers' and two household members' records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
7There were no physical exam records available for Child #3 and Child #4, who had been in attendance for over four months.
The record of Child #1 and Child #2, who had been enrolled for over five months, did not contain written parental authorization for emergency medical care.
The record of Child #1, Child #2, Child #3, and Child #4 did not contain the name, address and telephone number of a second designated person to contact in case of an emergency if the parent cannot be reached.
The record of Child #1, Child #2, Child #3, Child #4, and Child #5 did not contain the parent's written acknowledgment of the receipt of the required information. The children had been in attendance for over five months.
There was no emergency preparedness and response plan available at the time of inspection.
There was no documentation of the emergency evacuation and shelter-in-place drills that have been practiced in the last year.
Child #5, 12 months old, last documented immunization was over eight months ago.
