During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 02/04/2025 and completed on 02/10/2025, as a part of the licensure period. The on-site inspection began at 10:15am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were seven children present, two caregivers, and one volunteer; the point total was 19. The inspector reviewed five children?s records and two caregivers', and one household member's records on-site. Additional information was reviewed electronically on 02/10/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
(Repeat Violation) The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There was no physical exam record available for Child #2, who had been in care for over 6 months.
Plan of Correction: The missing immunizations and physical examination will be requested.
Standard 8VAC20-800-210-A
Caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Caregiver #1 did not obtain any training hours in 2024.
Plan of Correction: The provider will ensure caregivers obtain 16 hours of annual training.
Standard 8VAC20-800-30-A
The provider shall ensure compliance with the terms of the current license issued by the department.

Child #1 was 4 months old when the child started attending the family day home. Child #3 was 5 months old when the child started attending the family day home. Both children were in care at the time of the inspection on 02/04/2025.

The terms of the license indicates the family day home may serve children between the ages of 0years, 6 months through 8 years, 11 months.
Plan of Correction: The provider requested to decrease the ages on the terms of the license.
Standard 8VAC20-800-60-B
(Repeat Violation/Systemic Violation) A child's record shall contain the required information.

The following information was missing from the following children?s records:
Child #1, enrolled for over a month, was missing written authorization for emergency medical care, and documentation of proof of identity and age.
Child #2, enrolled for over six months, was missing two emergency contacts, the child?s & parents? address, the child?s physician information, written authorization for emergency medical care, and documentation of proof of identity and age.
Child #3, enrolled for 15 days, was missing written authorization for emergency medical care.
Child #4 and Child #5, enrolled for 12 months, were missing written authorization for emergency medical care.
Plan of Correction: The missing information will be obtained for each child.

(Repeat Violation) The provider shall obtain the parent's written acknowledgement of the receipt of the information regarding family day home operations and administration.

There were no parent acknowledgements available for five children who were provided care between February 2024 and January 2025.

The provider shall obtain the parent's written acknowledgement of the receipt of the information in this section.

The records of Child #1, Child #2, Child #3, Child #4, and Child #5 did not contain the parent's written acknowledgment of the receipt of the required information. The children had been in attendance between 15 days and 12 months.
Plan of Correction: The missing information will be obtained for each child.

(Repeat Violation/Systemic Violation) The family day home shall have a written emergency preparedness and response plan.

There was no emergency preparedness and response plan available at the time of inspection.
Plan of Correction: The required emergency preparedness plan will be created.
Standard 8VAC20-800-830-A
(Repeat Violation) The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.

The emergency evacuation procedures were not practiced in the month of December 2024 or January 2025.
Plan of Correction: The drills will be practiced monthly.

Before attendance, documentation of immunizations is to be obtained for each child.

There was no documentation of immunizations for Child #2, who was in care during the inspection and had been in care for over six months.
Plan of Correction: The missing immunizations and physical examination will be requested.
