During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on 06/05/25 and completed on 06/12/25, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to administration and emergencies. The on-site inspection began at 1:40pm and ended at 2:20pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 10 children?s records on-site. Additional information was submitted electronically on 06/12/25. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
(Repeat Violation) The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

The physical examination for Child #5 was completed more than two months after the child's first day of attendance.
Plan of Correction: Already corrected. Families will be reminded that a physical examination is due within 30 days of the child's attendance.
