Inspector Notes

An unannounced 60-day inspection was conducted on 9/5/23 from 10:40am to 12:00pm with the provider. The 60-day inspection is to observe standards that could not be observed during the initial inspection. There were four children in care, ranging in age from nine-months to 23-months-old, for a total of 13 points. Children were observed eating lunch, washing hands, and having free play. Four child records were reviewed. The provider has current certification in CPR and First Aid. Three medications were reviewed and the provider has current certification in Medication Administration Training (MAT). Lunch served was pasta with spaghetti sauce and turkey meatballs, broccoli, cut apples and grapes, and milk. The first aid kit, flashlight, weather band radio, and carbon monoxide detector were observed. The emergency drill log, attendance, and menu were also reviewed. Standard 8VAC20-800-100-A
Based on review of four child records, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance for one child. Evidence: The record for Child A, with an enrollment date of 7/5/23, did not contain documentation of a physical.
Plan of Correction: Physical examination will be obtained for Child A.
Standard 8VAC20-800-50-A
Based on review of documentation, the provider did not keep a written record of children in attendance each day. Evidence: On 9/5/23 there was no written record of attendance for the day.
Plan of Correction: Attendance sheet will be maintained.
Standard 8VAC20-800-60-B
Based on review of four child records, the provider did not obtain all of the required information for child records. Evidence: The records for Child A and Child D did not contain the name, address, and telephone number of each custodial parent?s place of employment. The records for Child A, Child B, Child C, and Child D did not contain parent?s signed acknowledgement of the receipt of the information required to be provided in writing to parents before the child?s first day of attendance.
Plan of Correction: The records will be updated.
Standard 8VAC20-800-700-A
Based on review of medication and medication forms, the provider did not obtain written authorization from a parent in order to give prescription medication to a child. Evidence: There was no written authorization from the parent for the two emergency medications on site for Child D.
Plan of Correction: Written authorization from parents will be obtained.
Standard 8VAC20-800-710
Based on review of medication, the provider did not ensure that medication administration requirements were met in that only those drugs that were dispensed from a pharmacy and maintained in the original, labeled container may be administered by the provider. Evidence: The prescription emergency medication on site for Child D was not maintained in the original, labeled container.
Plan of Correction: Medication will be in the original containers.
