The center is required to obtain a completed sworn statement prior to the employee's first day of employment. The sworn statement for S4, who is currently working, was dated 1 day after their first day of employment.
Plan of Correction: We will be sure to make sure all dates are correct on all forms.

The center must request a search of the central registry prior to the employee's first day of employment. S3, who had been employed for 13 days, did not have a central registry search requested.
Plan of Correction: Although CRS was returned in 30 days per regulation, we will be sure to submit CRS on the first day of employment.

Plan of Correction: We will be sure to train all teachers to dispose diapers by not touching the lid but by stepping on trash can petal to allow lid to be opened.
