Inspector Notes
An unannounced monitoring inspection was conducted on February 6, 2024 between the hours of approximately 10:20 a.m. and 11:15 a.m. There were 5 children present with the Provider for a total of 16 points within the supervision guidelines. The children were observed engaging in free play. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and caregiver records, fire drill log, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 7Evidence: A crib being used by Child #4 had a swaddle blanket laid across the mattress instead of a tight-fitting bottom cover.
Evidence: At the time of inspection, the family day home did not have a record for Child #3 (start date: 01/08/2024) and Child #4 (start date: 02/06/2024).
Based on record review, the family day home did not have all information for 2 out 4 children records.
Evidence: Child #1?s (start date: 10/30/2023) and Child #2?s (start date: 02/06/2024) records did not contain written authorization for emergency medical care.
Evidence: Child #1?s (start date: 10/30/2023) and Child #2?s (start date: 02/06/2024) record had a signed written information for parents model form. However, the provider did not complete the form with the required information.
Evidence: The records for Child #1 (start date: 10/30/2023) and Child #2 (start date: 02/06/2024) indicate that proof of identification was viewed by a third party and not the provider.
Evidence: The most recent immunization record for Child #1 (age: 21 months) was dated 05/08/2023.
Evidence:
1. A bottle belonging to Child #4 was not labeled with the child?s name and the date.
2. There were 2 infants in care at the time.
